Determine what needs returning

  1. Mick PIG Cylinder
  2. Strand Tray
  3. Lead Pouches & PIGS
  4. Braided Strand Tray & Tube Shield

Step 3.

Does the container contain brachytherapy seeds?

Yes

Survey per your site’s procedures:

if Surface Dose > 0.5 mrem/hr

  1. Contact Isoray Customer Service.

Surface Dose ≤ 0.5 mrem/hr

  1. Remove/cover the following from box:
  2. Yellow II Labels or White I
  3. UN2915 marking
  4. DOT Type A marking

Ensure the following is present on box:

  1. UN2910 label

Apply:

  1. FEDEX shipping label over previous label on top of box.

No

Remove/cover the following from box:

  1. Yellow II Labels
  2. UN2910 Label
  3. UN2915 marking
  4. DOT Type A marking

Apply:

  1. FEDEX shipping label over previous label on top of box.

Step 4.

Secure the box with tape and schedule a pickup with Fedex
